August 15, 2009 for review by the Board at the October 2009 meeting – if logs are approved, clinical examination cases are due in the Board Office by April 9, 2010 – the clinical examination date would be Friday, May 21, 2010 at the Ritz-Carlton-Palm Beach in Manalapan, FL
March 15, 2010 for review by the Board at the May 2010 meeting – if logs are approved, clinical examination cases are due in the Board Office by September 10, 2010 – the clinical examination date would be October 23, 2010 at the Grand Hyatt San Francisco in San Francisco, CA
